About Us

Community Health services vary from centre to centre and include family health services, mental health services, child health services, HACC, community health nursing services and confidential counselling. Referrals to self-help groups. Some Community Health Centres offer access to Home and Community Care (HACC) programs. HACC programs are a joint state and federal government initiative which provides frail aged people and people with a disability access to practical services which assist them to continue living in their own home. Services which may be available to eligible clients include domestic assistance, social support, home nursing and personal care, respite care (in home and centre - based), home maintenance or minor modifications, allied health services (such as podiatry, dietetics, physiotherapy and speech pathology) and referral to other support services where needed (such as Meals on Wheels). Also Womens Health, O.T. and Palliative Care. Fees apply to certain HACC services, but special arrangements may be made for people who cannot afford these costs.
