About Us
Osborne Park Older Adult Mental Health is a specialised service for people who are 65 years or over, reside in the Osborne Park catchment area and have a mental health disorder or are experiencing difficulties related to dementia, e.g. severe behavioural problems that cannot be managed by other services or family.
Those people who meet the referral criteria are usually assessed in their own home by members of the multidisciplinary team. Once individuals' needs are identified by the team they may be referred to one or more of the following older adult mental health programs:
- community support
- outpatient appointments
- therapy services
- carer support
- admission to the Acute Admission Unit.
The 24 Bed Acute Admission Unit is for clients requiring mental health assessment, management and stabilisation.
Referral Process: Written referrals from general practitioners or mental health professionals should be addressed to the Duty Officer of the day. Download the Mental Health Referral Form.
