About Us
Oseca operates Peel Health Hub, a ‘no wrong door’ facility for the health needs of young people, adults, families, and carers in the Peel region.
We have developed a Model of Care to offer Clinical Care Coordination (CCC) to individuals who need help but are unsure which services to access.
Our GP-supported approach focuses on improving mental, social, and physical health to ensure comprehensive care for all our clients.
About the program
Oseca is the lead agency of the Peel Health Hub, a central location for the health needs of young people, adults, families, and carers in the Peel region. At Oseca, we have developed a Model of Care to offer Clinical Care Coordination (CCC) support, GP and nursing services to individuals who need help but are unsure which services to access. Our qualified professionals provide a safe, confidential point of contact, helping to create care plans, initiate treatment, and connect clients with both internal and external services.
Supports available
Oseca provides a range of services at the Peel Health Hub, focusing on coordinated care and support for individuals who may not know what services they need. Supported by GPs and nursing staff, the Clinical Care Coordination (CCC) model ensures clients receive personalised, seamless care to improve their health and wellbeing.

- GP & Nursing services (limited)

- Access to health and welfare services within the Peel Health Hub
- Assistance with accessing external services
- Safe and confidential first point of contact
- Initiation of treatment with client consent
Referral pathways
Our Oseca PHH team provides primary healthcare and advanced clinical access interventions to young people and families in the Peel Region. This ensures early comprehensive assessment, initial care-planning and timely care.
We operate a ‘no wrong door’ policy, so if you or someone you know needs help please contact us by phone, email or in person
Referrers (inclusive of external GPs) should contact the Clinical Care Coordinator triage officer at PHH, to discuss potential referrals in the first instance.
