Supports patients who have chronic conditions, to access a multi-disciplinary allied health service, including telehealth and individual care coordination.

The ICDC Care Coordinators receive referrals from GPs and are responsible for ensuring the patient receives appropriate coordinated care, including access to a multi-disciplinary team-based service that provides a wraparound patient-centred approach.
