WHO WE ARE

Aus-Ability WA is a smalll practice run by Accredited Social Workers with years of experience whose mission is to make a difference in the lives of others. Staff undergo continuous extensive training.  We believe that empowering others begins by empowering ourselves.  We promote continuous education and training as a part of our staff development. We work closely with the National Disability Services (NDS) to ensure we have the most up to date information, resources, and training.

Support Coordination – Specialist Support Coordination

Getting the most out of your NDIS plan sometimes requires a little help. Often the system can be confusing and overwhelming especially when you’re busy living day-to-day with a disability or caring for someone who does.

 

What Is Support Coordination Level 2 – Coordination Of Supports?

The role of a Support Coordinator is to work with you, get to know you and your goals so you can get the very best out of your NDIS plan. A Support Coordinator provides unbiased recommendations on the types of support you might like to access, and where you can best access them. The choice and control is all yours. A Support Coordinator can help provide you with more choices.

At Plan Review Stage, A Support Coordinator Can Help You:

  • Prepare for your planning meeting.
  • Set and refine goals.
  • Set objectives and strategies.

 

 

Once you have a plan, a Support Coordinator can help you:

  • Manage your supports within the budget parameters of your plan.
  • Ensure you have genuine choice and control of service providers.
  • Provide you with the tools to manage any issues that may occur with your service provision.
  • Ensure you can address issues or barriers in accessing service delivery within existing funded supports in the first instance.
  • Request additional funds if you have a change in circumstances.

 

What is Specialist Support Coordination?

A Specialist Coordinator delivers a higher level of support than that of a Support Coordinator. The focus of a Specialist Support Coordinator is to get to know you and focus on reducing complexity in your life by helping overcome immediate and/or significant barriers in the plan implementation.

The Specialist Coordinator helps:

  • Address complex barriers that affect access to supports
  • Design a service plan for their support needs, where appropriate.

A Specialist Support Coordinator in Aus-Ability WA is a qualified Allied Health Professional with appropriate experience to deliver the expert approach which is needed to address complex support needs and/or risks. Specialist Support Coordinators work with you and your support network to:

  • Identify and understand the barriers impacting ability to access, use and maintain supports.
  • Find and try solutions to address these barriers.
  • Establish a process to overcome these barriers, including prioritising factors that need to be addressed first and what supports need to be in place immediately.

A Specialist Support Coordinator Can Help You:

  • Ensure all services work effectively and cohesively together to help achieve goals in the plan.
  • Establish processes to enable communication, cooperation and collaboration within the participant’s network of funded and non-funded supports.
  • Helps resolve issues and unexpected situations.
  • Ensures a consistent delivery of service and access to relevant support in a crisis or unplanned event.

Once a service is designed the Specialist Support Coordinator will coordinate the supports. The cost of Specialist Support Coordination and Support Coordination is often covered by the funds in your NDIS plan.
