About Us

The team at Realise Ability is made up of local therapists, therapy assistants, support coordinators and support workers. 

What we do?

We can help people test their eligibility and gain access to the NDIS. 

We help people with their planning meetings and and help review decisions. 

We help with annual plan reviews.

We help people understand their plan. 

We help people to maximise and use their plan. 

We help people find services in town and the right services for them.

We help people set their goals.

We provide support coordination.

We provide therapy and support services to help people achieve their goals. 

FEES - All our services are for people with NDIS plans and/or with GP management plans and/or people eligible for private health rebates. Our fees are in accordance with the NDIS price guide. 

How We Roll!

Our team work within a social model. We provide a wrap around and holistic service that help clients work towards goals that are important to them, in environments that are important to them.

The therapist is important, but not as important as the client and the rest of their team.  The client is the expert and their life and their goals dictate where they practise their skills.

The Client chooses the goals – therapists help clients work out which skills and strengths they need to develop to achieve these goals and together they can write a plan for their team.

The Client chooses where they want to use the skills – their team will work with the client in these places to ensure their new skills are useful and functional.

The client's team may include family, teachers, friends, coaches and anyone else who plays a role in their life.  Every now and again, they will need to check back in with their therapists to make sure they are on track and get some new ideas.

Our psychologist will work with the client around goals related to social skills, anxiety, resilience, change, behaviour and relationships.  They will not provide counselling, but give clients ideas and strategies to explore and practise with their team.

The occupational therapist looks at daily living, play, work and employment, social skills, behaviour and relationships.  The plan they build with the client will include strength, coordination, organisation and ability to use their new skills everyday.

Speech pathologists assist people to develop the communication, eating and social skills they need to have successful relationships and to build their literacy and numeracy.  A speech plan will involve practising speech, language and communication in a variety of environments.

A dietitian will help the client's team identify methods for maintaining health and well being through diet and activity.  Plans created by a dietitian may involve the team supporting the client with shopping, cooking and exercise.

Strength, coordination, fitness and mobility are areas where physiotherapists may guide the client's team to plan and support fitness, sport and physical activities with them.

Realise Ability can build and support the client's team, to support them if they have a goal they want to achieve.  This may be at home, at the beach, in the pool, in sport, at school or anywhere else that is important to them!
