About Us
Promotes self-management skills to people living with chronic conditions through a virtual service as required, utilising a modified Flinders My Health Plan.
Eligibility to the service is for those clients with a diagnosis or at risk of chronic disease who require additional support. The service provides integration and centralisation of referrals to other WAPHA funded projects such as Integrated Chronic Disease Care (ICDC), nurse practitioners and telehealth.
